Forms and resources for current students:

• Academic Warning Student Worksheet - Second Warning (pdf)
  Use this form if you've been placed on a Second Academic Warning. You will be required to meet with an academic advisor to complete this worksheet.
• Academic Warning Student Worksheet - Third Warning (pdf)
  Use this form if you've been placed on a Third Academic Warning. You will be required to meet with an academic advisor to complete this Academic Reinstatement Petition.
• Admissions Application (pdf)
• Application Fee Waiver 
  Use this form if you are unable to pay the $25 application fee for admission to COCC.
• Application for Certificate or Degree
  Use this form to apply for any degree or certificate that is not your declared program of study if you have enrolled in the final course(s) for your degree or certificate.
• Challenge Course (pdf)
  Use this form if you feel that you have the knowledge and experience similar to a particular course and would like to challenge that course for credit.
• COCC Academic Transcript Request (pdf)
  Use this form if you would like to request OSU or Continuing Education/Non-Credit transcripts only.
• Degree Status Change Form
  Use this form if you would like to change your degree seeking status.
• Incomplete Grade Contract (pdf)
  Use this form if you are requesting an Incomplete Grade due to extenuating circumstances that would prevent you from completing your class.
• Legal Name Change Form 
  Use this form to change your legal name and have your COCC student records updated.
• Non-Disclosure of Student Information Request Form (pdf)
  Use this form if you do not want COCC to release directory information to anyone. This form prohibits COCC faculty and staff from discussing any student business over the phone and all business will be conducted in person only.
• Non-Disclosure of Student Information Request Removal Form (pdf)
  Use this form if you would like to remove your request for Non-Disclosure and allow directory information to be released.
• Release of Information Request Form
  Use this form to grant COCC permission to release your information to designated individuals.
